Aging Out: Case management for Medicaid Waiver clients that utilize the Aged and Disabled Adult Waiver Services. This program includes coordinating home health services and consumable medical supply services, completing comprehensive assessments, collaboratively develop care plans with clients and their families. The program is managed and facilitated through the Florida Department of Elder Affairs in Tallahassee, FL.
Eligibility Requirements for the Aged and Disabled Adult Waiver

Age 21 or older

Cognitively Intact - defined as oriented to time and place and able to display independence in daily decision making, comprehension and problem solving ability.

Medically Complex - individuals with chronic debilitating diseases or conditions of one or more physiological or organ systems that make the individual dependent upon medical, nursing or health supervision or intervention.

Technology Dependent - individuals requiring medical apparatus and procedures to sustain life.
Family Support Coordination: In partnership with the Children’s Board of Hillsborough County and Tampa JCC Preschool North, our Family Support Coordinator provides assistance to students and their families through support plans, family team meetings, classroom observations and referrals to community resources.
Information and Referral: As an agency listed with ACCESS, TJFS refers services made available by public and private providers, after a brief assessment of client needs to facilitate appropriate referral to community resources.
