SENIOR CARELINK Can Be There… When You Can’t. 
TJFS’s Senior CareLink Program provides:
- Comprehensive Assessment: Provide in-home evaluation to identify any problems and concerns so that solutions can be proposed and discussed
- Individualized Care Plan: Make recommendations for services, areas for further exploration and monitoring
- Community Referrals: Assist in obtaining service providers and in-home care
- Monitoring: Screen, arrange and monitor services
- Counseling, Education, Support: Counseling and support for senior adults. Provide direction and guidance for issues and concerns as well as family consultation and support
- Crisis Intervention: Stabilization of crisis situations
- Advocacy: Serve as a liaison with community providers
- Peace of Mind: Communication with family members
